All school volunteers are required to authorize a criminal offender records check at least once every three years. Forms are available at each school where the volunteer must present a government issued photographic identification (valid driver's license, passport, etc.) along with a completed form. You can complete the form provided at the school or you can download the form with the link below to prepare before bringing it to your child's school. DO NOT MAIL FORM - it must be presented in person by the individual along with the original ID.
The turnaround process time by the Criminal History System Board board varies, so please allow at least two weeks prior to planning on volunteering, chaperoning, etc. At times, it may be longer.
