Substitute Information

EMPLOYMENT PROCESS FOR SUBSTITUTES

Please complete the following to assist us in processing your employment paperwork. All documents are listed below.

  • Employment Application
  • Current CPR Certification: (School Nurse only) 
  • W-4
  • I-9 Form:  Please refer to the I9 Form for the list of acceptable documents for employment verification.  Please see the PDF document below referring to the I-9.
  • CORI: Massachusetts General Law requires criminal background checks.  A photo identification is also required. 
  •  Direct Deposit Form (optional): A voided check needs to be attached to the completed form.
  • Retirement Enrollment: OBRA Form is required for all temporary and some part-time employees.
  • Substitute Preference Sheet
  • Substitute Interview Form, top section only.
  •  Summary of Conflict of Interest Law:  Please read the Summary and complete the Acknowledge of Receipt.

 

Once you have completed all of the above paperwork, please bring your paperwork along with a license and two forms of I.D. (as listed on the I-9 acceptable documents list), to the Hudson Public Schools Administration Building at 155 Apsley Street Hudson, MA.  Once your paperwork is received you will be contacted by a building principal for an interview.

NOTE:  It is important to keep personnel records up to date.  Please notify the Human Resource office if changes occur on the information that you have submitted to our office.
