HUBERT KINDERGARTEN REGISTRATION 2012-2013
How to register for next school year:
All children living in Hudson who will be five years old on or before August 31, 2012 are eligible to enter kindergarten in the fall of 2012. There are no exceptions regarding entrance age requirements.
1. Parents/Guardians of eligible children should follow the directions below:
Ø Print and complete the Hudson Public Schools Enrollment forms, Check List of required forms
AND
Ø Print and complete the Hubert Kindergarten Packet
*Children currently attending one of Hudson Public Schools preschool programs at Hudson High School or Mulready:
Ø You will be receiving a registration packet from your child’s Hudson Public Schools Preschool teacher.
Parents who cannot access the Hudson Public School enrollment forms and Hubert Kindergarten Registration Packet may call the school at (978) 567-6130 to request both packets.
v Required Medical Documents
1. Physical Exam: copy of child’s most recent physical exam from 2011-2012
2. Vision Screening: documentation of distance acuity AND stereopsis
3. Lead Screening: documentation of date AND results
4. Immunization Record: Must be up-to-date showing:
§ DTaP/DTP 5 doses
§ Polio 4 doses
§ Hepatitis B 3 doses
§ MMR 2 doses
§ Varicella 2 doses
· Please provide any information regarding your child’s drug and/or food allergies. If food allergy exists and an Epi-pen is prescribed, please submit 2 wallet size photos of child.
· If you do not have a copy of your child’s immunization record, vision screening or physical at home, you may need to obtain one from your doctor. Please be sure your child’s physical exam and immunizations are up-to-date, and that a lead screening has been done, as these are required for entry into school. Check with your doctor if you have any questions.
2. Registration appointment:
Ø Once you have completed the forms and have in your possession the required documentation, please email your name, your child’s name, your child’s date of birth, your address and phone number to kregistration@hudson.k12.ma.us, in order to be assigned a registration appointment.
§ Registration appointments will be held on Wednesday, April 11 and Thursday, April 12 and will be assigned on a first come, first served basis. The deadline for appointment requests is Wednesday, April 4.
§ Registration appointments will be held at the Hudson Public Schools Administration Building, 155 Apsley Street.
· The registration appointment will consist of a 15-minute health interview between a Hudson Public Schools’ nurse and parents/guardians regarding their child, and the submission of the required forms and documents.
· It is not necessary to bring the child you are registering.
